의료자원은 한정되어 있으며, 이는 명확히 정의되어 적절하게 분배되어야 한다. 따라서 장기재원인 재원일수 상위열외군의 요인을 분석하고 이를 최소화시키기 위한 노력은 건강보험 재정 안정과 의료기관의 효율성 및 의료의 질 관리에 매우 중요한 요소이다. 연구 목적은 재원일수 상위열외군을 선정하여 병상규모별 질환별 재원일수 상위열외군을 분석하여 그 결과를 국가(보험자)와 병원의 재원일수 지표 관리의 참고 자료로 이용할 수 있도록 하는데 있다. 연구에 사용된 자료는 질병관리본부의 퇴원손상심층조사로 2005년부터 2008년까지 700,091건의 퇴원자료를 이용하였다. 비열외군과 열외군의 인구사회학적 특성, 임상적 특성과 기타 관련 특성을 비교하기 위해 t-검정과 χ²-검정을 시행하였고, 각 변수와 열외군과의 관계를 분석하기 위해 로지스틱 회귀분석(logistic regression)을 하였다. 열외군의 환자구성은 2.2%로 평균재원일수 102.8일이고 총 재원 일수의 20.0%를 차지하였다. 열외군의 평균재원일수는 102.8일로 전체환자의 평균재원일수 11.3일보다 91.5일 길었다. 2005년에 비해 2008년의 경우 비열외군 환자의 비율이 높았다. 이는 개별 의료기관에서 재원일수 단축을 위해서 노력하고 있는 것으로 보이지만 아직도 열외군 환자에 대한 종합적인 관리가 필요한 것을 알 수 있다. 열외군의 재원일수 관리가 보다 집중적이고 구체적이어야 한다. 장기재원환자인 재원일수 열외군 환자 관리활동을 통해 재원일수를 단축하는 것과 동시에 의료의 질을 모니터링 하는 것이 중요하다. 재원일수의 단축은 의료의 질이 동일하거나 더 나은 결과를 도출하는 경우에만 진행되어야 할 것 이다.
Background: It is important for the hospital mangers and policymakers to control the length of stay (LOS) outliers. The aim of this study was to find factors that explain length of stay outliers. Methods: This study targeted 700,091 cases based on the system data from In-depth Injury survey data by the Korea National Hospital Centers for the Korean National Hospital Discharge In-depth Injury Survey data from 2005 to 2008. The dependent variable, LOS outliers, was calculated for each diagnostic group using a trim point defined for each year by the geometric mean plus two standard deviations. Logistic regression model were used in the analysis. Results: In inpatient episodes analysed we found a proportion of 2.2% high LOS outliers, accounting for 20.0% of total inpatient days. The proportion of LOS outliers slightly decreased between years 2005 and 2008. Conclusions: As high LOS outliers represent an important proportion in the total inpatient days, this should be seen as an important alert for the management of hospitals and for national health policies. There is a need for a different policy approach to managing the length of hospital stay so that hospitals can maintain profits and perform their inherent role of providing good-quality treatment.
